1. Acceptance
These Terms of Service ("Terms") govern your use of the patient portal and related digital services (the "Platform") provided by the clinic that referred you to this account. By creating an account, accepting an invite, or booking an appointment online, you accept these Terms.
If you are accepting these Terms on behalf of another person — including as a substitute decision-maker — you represent that you have the authority to do so.
2. The Platform is not medical advice
The Platform is a tool the clinic uses to manage scheduling, communications, records, and billing. It is not a substitute for direct medical care. If you are experiencing a medical emergency, call 9-1-1 or go to the nearest emergency department.
3. Eligibility and accuracy of information
You must be 16 years of age or older to register for an account on your own behalf. Patients under 16 may receive care at the clinic; their account will be created and managed by a parent or substitute decision-maker.
You agree that the information you provide during registration and updates is true and accurate to the best of your knowledge. Providing false information may delay or prevent care and may result in account suspension.
4. Account security
You are responsible for keeping your login credentials confidential and for all activity that takes place under your account. If you suspect unauthorized access, contact the clinic immediately. We require multi-factor authentication for staff accounts and may require it for patient accounts in high-risk scenarios.
5. Booking, cancellation, and no-show policy
Online booking is a request, not a confirmation. The clinic will confirm or adjust your appointment based on clinical priority and availability. Each clinic publishes its own cancellation and no-show policy; you accept that policy when you book. Repeated no-shows may result in restricted booking privileges.
6. Communications
We communicate with you about appointments, results, and care follow-ups using the contact methods on file. Standard messaging rates may apply for SMS. You may withdraw consent for non-essential communications at any time; clinically essential communications (for example, urgent results) will continue.
7. Permitted use
You may use the Platform only for lawful purposes related to your own care (or, where authorized, the care of someone you represent). You agree not to:
- Attempt to access another person's record.
- Reverse-engineer or interfere with the Platform.
- Upload malicious software or content that is illegal, harassing, or unrelated to your care.
8. Content you upload
When you upload documents, images, or messages to the Platform, you grant the clinic a license to use that content for your care, for required regulatory purposes, and to operate and improve the Platform consistently with the Privacy Policy. You retain ownership of your content.
9. Suspension and termination
We may suspend or terminate your account if you breach these Terms, if there is fraud, or if the law requires. You may terminate your account at any time. Termination of your account does not erase your clinical record; the clinic retains it for the period required by law (typically 10 years; see the Privacy Policy).
10. Disclaimers and liability
The Platform is provided on an "as is" and "as available" basis. To the extent permitted by Ontario law, the clinic and the Platform disclaim implied warranties of merchantability and fitness for a particular purpose. Nothing in these Terms limits or excludes liability that cannot be limited or excluded under applicable law (including liability for death, personal injury, or fraud).
11. Changes
We will update these Terms as our practices change or the law requires. The effective version is shown at the top. We will ask you to re-accept these Terms when the change is material.
12. Governing law and disputes
These Terms are governed by the laws of the Province of Ontario and the federal laws of Canada applicable in Ontario. Disputes will be resolved by the courts of Ontario, except that you retain any consumer-protection rights the law gives you that cannot be waived.
